Who Can Submit a Nomination?
All nominations must be submitted on behalf of the patient.
To preserve the spirit of the Save-A-Smile program, nominations must be submitted by someone advocating on behalf of the individual. Self-nominations are not accepted.
How to Nominate a Patient
Please complete the nomination form and include a thoughtful description of:
The individual being nominated
Their need for orthodontic treatment
Why you feel they would be a deserving recipient
How treatment could positively impact their life
Once the nomination has been reviewed, a member of our team will contact the referring office with additional information.

At Johnsonlink Orthodontics, we believe a healthy, confident smile can change a life.
Through our Save-A-Smile program, we provide full orthodontic treatment at no cost to a deserving patient in need. This program was created to support individuals who would benefit from orthodontic care but may not otherwise have access to treatment.
We are honored to partner with local referring doctors to help identify patients who may experience a meaningful, life-changing impact through orthodontic treatment.
