At Johnsonlink Orthodontics, palatal expanders are used to gently guide jaw growth and create the space needed for a healthy, balanced smile. This early treatment allows us to address bite concerns and crowding at the right time, using modern techniques that are more comfortable and precise than ever before.

Traditional Metal Expander (RPE)

A traditional palatal expander, also known as a Rapid Palatal Expander (RPE), is a custom-made appliance used to gently widen the upper jaw. It works by gradually creating space in the palate to improve how the upper and lower teeth fit together. This type of expander is most often recommended when the upper jaw is too narrow, which can lead to issues such as crossbite or crowding. The expansion process is carefully guided by your doctor and requires small, regular activations using a key until the desired result is achieved.

How the Expander (RPE) works

The expander is placed on the upper back teeth and works by gradually widening the palate over time. Each activation gently moves the upper jaw in small, controlled steps. This process is carefully planned by our doctors to ensure safe and effective results.

You will be shown exactly how to activate the expander, and our team will make sure you feel comfortable before you leave your appointment.

How often do I turn the Expander?

Dr. Johnson or Dr. Lingenbrink will give you specific instructions, but most patients will:

  • activate the expander once or twice per day

  • follow a consistent schedule (same time daily is best)

  • complete all turns as directed for proper results

If you ever misplace your activation key, just contact our office, we’re happy to replace it!

How to activate your Expander

Your assistant will demonstrate this in office, but in general:

A small key is inserted into the center of the expander and gently turned until the next hole appears. The key is then removed, completing one activation. (See Fig 1-4 on the left for reference)

What to expect

It is normal to experience a short adjustment period when the expander is first placed.

You may notice:

  • slight pressure on the teeth or roof of the mouth

  • temporary changes in speech

  • mild soreness when chewing

  • increased saliva for the first few days

A small space may also appear between the front teeth during treatment, this is completely normal and often closes naturally over time. Most patients adjust within a few days.

Eating and Hygiene tips

Food can easily get caught around the expander, so good cleaning habits are important. We recommend brushing your teeth, gums, and the expander thoroughly, rinsing with water after meals, and using mouthwash if recommended by your doctor, while avoiding sticky or very hard foods.


Invisalign Palatal Expander (IPE)

The Invisalign® Palatal Expander (IPE) offers a modern, more comfortable alternative to traditional expanders. Instead of metal and screws, it uses custom 3D-printed appliances designed to gently guide jaw expansion. There is no need for manual turning with a key, instead, patients progress through a planned series of expanders as the palate gradually widens. This option is designed to be more comfortable, discreet, and convenient, while still effectively creating space and improving jaw alignment.
